Your refund will be in the form of a prepaid gift card.

Please note that because we are unable to control the quality of our products sold by unauthorized sellers, unless otherwise prohibited by law, the Crest Money Back Guarantee is not available for products purchased from unauthorized sellers, including unauthorized internet sites.

Requirements:

  • Crest offers a 60 Day Money Back Guarantee on our products.
  • The guarantee is limited to the original, end-user purchaser.
  • Require a valid proof of purchase which includes: purchase date, purchase price, retailer name and product purchased.
  • Crest reserves the right to verify information and to deny guarantee requests at its discretion in cases of suspected fraud or if the purchaser has abused the Crest Money Back Guarantee.
  • You must submit your guarantee request within 60 days of the date of original purchase.
  • Limit one refund per name, household, or address. Use of multiple addresses or P.O. boxes to obtain additional refunds is fraud and may result in prosecution.
  • Offer limited to legal residents of: United States who are at least eighteen (18) years of age.
  • Refunds are in the form of a prepaid debit card.

How to Submit:

  • To submit a guarantee request, please complete the electronic Crest Money Back Guarantee Request Form within 60 days of the date of original purchase.
  • The Crest Money Back Guarantee Request Form must be completed and include proof of purchase, which must detail the purchase date, purchase price, retailer name, and product purchased, in order to receive a refund in the form of a prepaid card. Reproduction, alteration, sale, trade, or purchase of this submission or proof of purchase is prohibited.
  • The Crest Money Back Guarantee Request Form must include the actual UPC/Barcode (a 12-digit number found on the bottom or sides of the packaging) from the Whitening product you purchased. Please ensure to include both codes if you are submitting for the Crest Club Store Whitestrips Twin Pack.
  • Proof of purchase must be obtained from the product purchased by you. No requests from groups, clubs, or organizations will be honored. We will not honor incomplete submissions. Not responsible for lost, late, or undelivered submissions.
